Take this short survey and help close the gaps for caregivers!

The New York State Office for the Aging (NYSOFA) and the New York State Caregiving & Respite Coalition (NYSCRC) are working together to pilot a Regional Caregiver Wellness and Respite Center (CWRC) to strengthen New York’s Lifespan Respite Care System. The CWRC aims to expand service provisions and delivery to caregivers in a six-county region (Clinton, Essex, Franklin, Hamilton, Warren, & Washington).

The purpose of this survey is to better understand the gaps in needed services for caregivers as compared to our previous needs assessment. The survey should take about five minutes to complete. Please submit your response by December 22, 2024. All responses will remain confidential.

No one understands caregivers like other caregivers. By taking part in these meetings, you’ll have the chance to develop meaningful relationships with others in your own community. You’ll even have the opportunity to continue your connections after the sessions have ended. See group schedule and register.
